A medical certification for a disability exception was required for applicants for citizenship, and the requirement no longer exists. However, the form is still available for use. To complete these forms, fill out the form as provided, then attach a certificate of medical certification stating the name, disability, and date of birth of each applicant in a copy, or electronic copy (see below), that a medical office has issued or will issue to an applicant, including documentation of previous disability. Note that you may not need to make any special entry for yourself on the application. Do not leave a space for the medical examiner to write that the form is being filed in the office of Mr. Jones, Medical Examiner, U.S. Department of Homeland Security, N.C. DEPARTMENT OF VETERANS AFFAIRS, P.O. BOX 940, CHATTANOOGA, TN 37, unless the naturalization officer orders you to do so.
