Section VI, Question 22.1 asks the medical certifying medical professional for a statement listing the factors the medical practitioner has used to evaluate whether impairment exists for each condition listed on the form. The medical certifying medical professional must indicate these factors, which are not required, if they are applicable. If the medical certification indicates an impairment for specific conditions listed on the form, only these conditions must be listed in the certification form. The form can request that the applicant provide a copy of the medical certification for disability exceptions (Form N-648) or a record of clinical observation (Form N-961).
